2010 Season Registration Information

Regular Registration runs from April 1st to May 31st. Registration fees are $125 with discounts for 3 or more children in the same immediate family.

To register fillout the CYSA membership form (link on right). The CYSA membership form is available online as a fill-in form. If you fill out on line print 2 copies to mail or bring to a registration walk-in. Mail forms to P.O. Box 410 Wilton, CA 95693

Filling out the CYSA Membership Form:
Complete the player information, primary guardian, and secondary guardian sections. Most important, the parent/guardian MUST sign the Medical and Liability Release section of the form at the bottom right of the form. This form must be signed with a "Wet Ink Signature" must be in color ink (Blue, Red), not black.
Required Documentation:
Birth Certificates - If your child is a returning Pleasant Grove player, there is no need to include a birth certificate. We should have it on file.
Player Picture – Please submit a small photo 1”x1” with the name of the player written on the back.
Uniform Ordering Form - Mark the appropriate size for shorts and jersey. Socks come in two sizes youth and adult.
Parent Sideline Ethics Agreement Form - Read the form, EGYSL has a zero tolerance policy. Sign and return with registration materials.
Special Request Form - Please fill this form out if the player has been playing up an age group.

Incomplete registrations will be not be processed and will be returned to the parent/guardian

Returning Players

  • Registration materials are mailed after April 1st to the address of record
  • If you moved since last season you will need to attend a walk-in registration
  • Include small player photo(name on back) and copy of birth certificate, unless already on file
  • Registration forms do not have uniform size information this year. We are working on a method to best assure correct sizes for your child.
  • Mail your completed registration form, check or money order, photo, birth certificate, parent agreement form, and uniform size to the club.

Walk in Registrations

Pleasant Grove Soccer Club sponsored registration dates. All times are 6:00 PM to 8:00 PM
  • Dillard Elementary:   Wednesday - April 7    Map
  • Pleasant Grove Elementary:   Tuesday - April 13   Map
  • Adreani Elementary:  Friday - April 30   Map
  • Edna Batey Elementary:     Wednesday - May 5   Map
EGYSL sponsored registration dates. All times are 10:00 AM to 2:00 PM
  • Harriet Eddy Middle School:   Saturday - April 24   Map
  • Harriet Eddy Middle School:   Saturday - May 15  

Bring a small photo 1 x 1 inch and a copy of birth certificate, unless birth certificate is already on file.

 

Late Registration

  • Late registration is from June 1st to June 30th
  • Late registration fees of $20 per child is applied

Space Available Registration

  • Space-Available Registration is from July 1st through August 31st.
  • Fees are the same as for Late Registration a $20 late fee is applied.
  • You will not be guaranteed a position on a team. Space available is exactly what it says. If there is room or if there are enough players to form a new team you will be contacted
  • Space available registration does not guarantee a place in the Pleasant Grove Soccer Club, you may have to play for another club in EGYSL
